Emergency and Health Information

Your cooperation in filling out the health and emergency cards at the beginning of the school year and updating them as needed will help us to keep our records up to date and ensure the safety of your child.  It is extremely important that we have your current address and phone number in case we need to reach you regarding your child.  Any changes in emergency information need to be made at the VIP Village office in person and by the Legal Guardian.  If insufficient emergency information is available, your child may not attend preschool until such information is furnished.  Any person authorized to pick up a child must be 18 years of age or older.  Adults picking up students must present a valid photo identification card-NO EXCEPTIONS. This ensures the health and safety of your child.  Children will not be released to adults not listed on emergency cards or without a valid photo ID.


In the event of an emergency and the child becomes ill or injured at school, staff will take immediate action to obtain medical treatment for the child.
